WELCOME TO TRI-STATE ORTHOPAEDICS & SPORTS MEDICINE!
We are pleased to offer "e-Chart"-- a secure online portal for our patients to login and provide necessary medical history information prior to their office visit. Patients can also access some of their individual medical information including a visit summary online.
SIGN-UP AND LOG-IN: If you are interested in accessing the e-Chart, you will need to login to the secure portal with your email address and a password. We must have your current email address in our practice records. Please make sure our office staff has your email information on file during your next office visit or email us at portal@tristateortho.com so we can update your account. Once we have your email address, you will then be able to click the "sign-up" link and register by inputting your email and creating a password. This process is required to access your medical information.
ACCESS:
With proper login, you can view your e-Chart information at any time. However, to change or update your medical information (including history, medications, etc.) you must have an upcoming appointment with Tri-State Orthopaedics in the next seven (7) days.
NEW PATIENTS: If you have an upcoming appointment within the next 7 days with one of our orthopaedic physicians, please take 10-15 minutes to complete your "Medical History Form" online. This will save valuable time on the day of your visit.
RETURNING PATIENTS: If you are a previous or returning patient of the practice and have already completed the medical history "bubble sheet" form in our office or provided the information on the phone to our pre-registration staff, you do not have to provide this information again.
However, if you have not completed this form previously, or if you have any changes since your last visit, you must update the information within 7 days of your next visit. For example, if you are taking any new medications or have any new allergies, medical conditions, or surgeries then please update this information online prior to your next appointment.
SUBMIT & SAVE: Once you are logged into your account, please remember when updating your medical information to click the "submit page" button (bottom of screen) for each tab (patient history, family history, social history, etc) or your information will not save.
The patient portal is fully enabled to use API functionality. If you are interested in using a third-party application to access the information in your patient portal, please send a request to portal@tristateortho.com and we will assist you.
